Ontario Health is inviting expressions of interest from palliative care health service providers for the role of Clinical Lead, Models of Palliative Care: Hospital. This role will provide strategic clinical leadership and evidence-informed advice to support implementation and adoption of the palliative care model of care for adults receiving care in hospital settings. The Clinical Lead will lead knowledge translation, stakeholder engagement, change management, implementation planning, workforce considerations, evaluation, and recommendations that respond to emerging system priorities. This work will advance the Health Services Delivery Framework and the goals of the Ministry of Health’s Provincial Framework for Palliative Care. see attachment
